The Peptide Boom Meets Reality
What a comprehensive Nature investigation says about the loudest wellness trend of the year. And why caution here is not a spoilsport position.
Peptides are the loudest promise in the wellness world. TikTok glow-ups, bodybuilding forums, longevity conferences, US politicians. Google searches for the term have risen from 1.3 million per month to 8 million per month within two years. In June 2024, Nature dedicated a news feature investigation to the phenomenon. The verdict is clear: enthusiasm has raced far ahead of the evidence.
As a health professional, you know the dilemma. Clients arrive with screenshots from Instagram Stories asking about BPC-157, MOTS-c, or the so-called Wolverine Stack. They have heard that peptides are supposed to be the answer to everything, from joint regeneration to fat loss. You are caught between the desire to work evidence-based and the pressure not to appear behind the times.
The Nature analysis shows: your caution is justified.
What Peptides Actually Are
Peptides are short amino acid chains, typically fewer than 50 amino acids. They are not a pharmacological footnote. Insulin, oxytocin, growth hormone, and the GLP-1 agents are all peptides. Around 100 FDA-approved drugs belong to this class, with a further 150 in clinical trials.
When wellness influencers talk about "peptides," however, they usually mean a different group of substances: BPC-157, MOTS-c, TB-500, and combinations such as the so-called Wolverine Stack. These substances are not approved, are sold in vials labeled "for research use only," and are used systematically outside any regulatory oversight.
What the Evidence Actually Shows
Nature examined the field with a sober eye. The findings are sobering.
BPC-157: Three pilot studies with a combined total of 30 participants. Flynn McGuire of the University of Utah is quoted in Nature with the finding that the human data are of such questionable quality that they can essentially be ignored. No long-term studies. No safety studies, not even systematically in animal models.
MOTS-c: Promising mouse data and a single Phase I study with 65 participants for dose-finding and 20 participants with obesity plus NAFLD. Lower glucose levels, indications of hepatoprotective effects. Phase II funding did not materialize and the company was wound down in 2023.
TB-500 (Thymosin-β4): A Phase III study as eye drops for dry eye missed the primary endpoint. Further applications remain preclinical.
The central message from researchers such as Matt Kaeberlein (University of Washington) and Pinchas Cohen (USC) is consistent: these substances may be biologically relevant, but what consumers are currently injecting themselves with is research material of undefined quality, not medicine.
The Purity Problem
A preprint analysis cited in the Nature investigation tested around 6,000 samples from 203 suppliers for purity and dose. More than 40 percent did not meet even basic standards. Of approximately 250 endotoxin-tested samples, 15 percent contained measurable quantities. Endotoxins can cause anything from fever and chills to septic shock.
Pinchas Cohen, one of the discoverers of MOTS-c, puts it plainly in Nature: what people were injecting themselves with was what he would typically administer to mice.
